TERMS AND CONDITIONS OF USE

This web site ("Site") and related services are provided by UnityPoint Health and its Affiliates, including those health care providers listed at the end of this document (collectively "UPH") to its patients subject to compliance with the terms and conditions set forth below. Please read the following information carefully. Your continued use of this Site will indicate your agreement to be bound by the terms and conditions set forth below. If you do not agree to be bound by these terms and conditions, promptly exit this Site.

I. Privacy

UPH believes the privacy and security of its patients' information is a key part of our vision for the Best Outcome for Every Patient Every Time. UPH will use your medical information as described in the Notice of Privacy Practices for the applicable UPH entity, including to provide you with health care services. UPH will maintain the privacy of your confidential medical information and will not disclose medical information about you unless you authorize the disclosure of your information or the disclosure of the information is permitted by law without your consent. Please review the applicable UPH entity's Notice of Privacy Practices for a thorough description of how we gather, use and protect your confidential medical information. All messages sent and received within MyUnityPoint that contain medical information are subject to state and federal laws governing the security and confidentiality of medical records.

II. MyUnityPoint Features

A. Electronic Messaging

MyUnityPoint should never be used for urgent matters. The anticipated turnaround time for response to electronic messages is two business days. If you believe you are experiencing an urgent or life threatening health condition, please call your physicians office, go to an emergency room of a local hospital or dial 911.

If your provider is out of the office or unavailable, messages sent via MyUnityPoint may be routed to other authorized caregivers within UPH in order to facilitate a timely response to your request.

UPH may send you messages via MyUnityPoint. It is your responsibility to monitor these messages and respond in an appropriate manner.

B. Secure Communications

All communications between you and UPH using MyUnityPoint occur over a secure connection. You are required to provide the UPH providers with a valid e-mail address, and it is your responsibility to keep this address updated if you change your e-mail account. You will receive Internet e-mail messages notifying you that new information is available in your MyUnityPoint account. Although these e-mail messages will not contain any confidential medical information, the notification that new information is available by accessing MyUnityPoint may be information that you would not want others to know. Please take this into account when providing an e-mail address or using MyUnityPoint. All of the confidential medical information available to you in MyUnityPoint is protected and securely maintained by UPH.

C. Activation Code and Password

When you initially enroll in MyUnityPoint, you will be provided with an activation code. The activation code you receive will give you the opportunity to create a personal login name (called MyUnityPoint username) and a password. Once this activation code has been used it will no longer be valid. It is extremely important that you keep your username and password completely confidential. Anyone with access to your username and password will be able to access your confidential medical information. That person will also be able to read your messages and send new messages as if that person were you. It is your responsibility to prevent disclosure of your login and password, and to change your password if you feel that its security has been compromised. UPH is not responsible or liable for disclosures of medical information obtained through unauthorized use of a user's password or the sharing of this information with individuals not legally authorized to view a patient's medical information. You may change your password at any time.

D. MyUnityPoint Alerts

1. Program Description

MyUnityPoint Alerts allows mobile subscribers to opt-in to receive alerts about your MyUnityPoint account through SMS text messages on your mobile phone.

2. Supported Carriers

MyUnityPoint Alerts are available on the following carriers:

Alltel, Appalachian Wireless, AT&T, Bluegrass Cellular, Boost Mobile, Cellcom, Cellular South, Centennial Wireless, GCI, Immix Wireless, Inland Cellular, IV Cellular, MetroPCS, Nex-Tech Wireless, nTelos, Sprint PCS, T-Mobile, U.S. Cellular, United Wireless, Verizon Wireless, Virgin Mobile USA, and West Central Wireless.

T-Mobile is not liable for delayed or undelivered messages.

3. Cost

MyUnityPoint Alerts is a free service from UnityPoint Health. However, message & data rates may apply from your mobile carrier.

4. Message Frequency

If you opt-in to receive MyUnityPoint Alerts, you will receive one message for each alert.

Alert options can be configured on the MyUnityPoint notification preference page. Options include notification when new information is added to your chart, a message is available from your care team, and reminders for upcoming appointments.

5. How to opt-out of our service

To opt-out of MyUnityPoint Alerts, text STOP at any time to 70189. An unsubscribe message will be sent to your mobile number confirming the cancellation, but no more messages will be sent from this program after your STOP command has been processed by our systems. In most cases this happens quickly, but in unusual circumstances may be delayed. If you have sent us a STOP command and have questions about whether it has been processed, please contact us using the information about Support/Help below.

6. Support/Help

For support or information about MyUnityPoint Alerts, text HELP at any time to 70189. You can also email us at MyUnityPointSupport@UnityPoint.org or call 1-877-224-4430.

7. Privacy Policy

At MyUnityPoint, the privacy of our patient information is very important to us. We will use your information in compliance with federal and state laws on the protection of patient information. E. Features

At this time, UPH anticipates that the MyUnityPoint web portal will provide the following services:

  • Ability to review, request or cancel your appointments
  • Ability to view selected portions of your medical record such as allergies, medications, histories, problem list and recent visit information
  • Ability to receive selected lab results
  • Ability to communicate with your health care provider via secure messaging regarding health concerns
  • Ability to request prescription refills
  • Ability to link to other sites that will provide you information

III. Information Does Not Constitute Medical Advice

UPH assumes no responsibility for how you use the information you obtain from this Site. You should consult with a physician or other health care provider regarding your own condition and how this Site content may or may not apply to you. You may be accessing your own medical information via the MyUnityPoint application and may wish to communicate with one of your health care providers about that or related information. If you have an emergency, or think you need to speak to someone urgently, please do not use the e-mail communication options provided to you by MyUnityPoint. Neither MyUnityPoint nor any other application available through this Site is a substitute for appropriate and timely contact or consultation with your physician. You should never change or stop any course of treatment prescribed by your physician without first consulting him or her.

IV. Termination of Access

UPH may restrict or terminate access to MyUnityPoint at any time. If access is restricted for planned maintenance of the MyUnityPoint system, reasonable efforts will be made to communicate downtime to users in advance. However, there may be reasons to restrict access to MyUnityPoint without advance notice. If your use of MyUnityPoint violates these Terms and Conditions, or if UPH decides to make changes to its patient chart tools or services, UPH may permanently terminate your access to MyUnityPoint.

V. Restrictions on Use and Ownership

All pages within this Site and any material made available for download are the property of UPH and/or its subsidiaries and affiliates. The content on the Site is provided for UPH and its patients only. The names of UPH and the entities listed at the end of these terms and conditions, as updated from time to time, and their associated corporate logos are the trademarks and service marks of UPH and/or the entities listed. All other trademarks, service marks, and logos used in this Site, including but not limited to MyUnityPoint and Epic, are the trademarks, service marks, or logos of their respective owners. Nothing on this Site should be construed as granting any license or right to use any trademark, service mark, or logo displayed on this Site without the express permission of UPH or the third party that may own the trademark, service mark, or logo.

VI. Accuracy and Integrity of Information

Although UPH attempts to ensure the integrity of the Site, it makes no guarantees whatsoever as to the correctness or accuracy of the Site. It is possible that the Site could include typographical errors, inaccuracies or other errors. In the event that an inaccuracy is identified, you should inform UPH so that it can be corrected. Information contained on the Site may be changed or updated without notice.

VII. Availability

MyUnityPoint may not always be available to you due to system maintenance, back-ups or failures. Access is provided on an "as is available" basis and you are not guaranteed access at any particular time.

VIII. Use of Cookies

"Cookies" are small bits of information generated by our web server and stored on your computer's hard drive. Cookies can "remember" what information a user accesses on a web page to simplify subsequent visits by the same user. We use cookies to recognize you as a repeat visitor, to store your MyUnityPoint ID and to provide certain features that make your use of the Site more efficient. These cookies are not required to use the Site in any manner and are only an option to make your browsing easier. If you would not like to receive cookies, you may decline by adjusting the browser settings on your computer.

IX. Confidentiality

Both Federal and State laws govern the confidentiality of patient health care records. UPH is dedicated to keeping your medical information confidential. Our employees are subject to strict standards for maintaining the confidentiality of your medical information. By allowing you access to your information, you must share in this responsibility.

X. Security

The Site places considerable emphasis on the privacy of medical information and compliance with State and Federal privacy laws. MyUnityPoint is configured to be secure from unauthorized access, but UPH and the Site cannot be held responsible for:

  • Absolute security of all electronic communication transmissions between patient and Health Care Providers;
  • Unauthorized disclosure resulting from a user not logging out of an active session;
  • Unauthorized disclosure resulting from a lost or stolen user ID and password;
  • Unauthorized disclosure resulting from information printed by the user from MyUnityPoint;
  • Unauthorized disclosure resulting from personal computer settings or installed software products that may compromise information security.
  • Similar events outside the substantial control of Health Care Providers or the Site.

XI. Links to Other Sites

UPH may create links on the Site to other web sites that we think may be of interest to you or provide further information. UPH makes no representations whatsoever about any other web site that you may access through this Site or any entity that may post its advertisements on this Site. When you access a non-UPH site, it is independent from UPH and UPH has no control over the content on that web site. UPH does not endorse or accept any responsibility for the content or use of any linked site. If you decide to access any third party web site linked to this Site, you do so entirely at your own risk.

UPH has not reviewed the privacy policies of any web sites linked to or from the Site, and you should exercise care when visiting any other web site. We would like to hear from you if you find any web sites linked to or from the Site objectionable.

XII. Your Right To Object

If you believe UPH is using your personally identifiable information in a way that is not consistent with the Terms and Conditions, you should contact us at once.
